Obiettivi
Dare allo studente gli strumenti teorici e pratici per conoscere le indicazioni in ambito diagnostico allo studio invasivo e non invasivo del cuore con le più moderne metodologie supportate dalle linee guida.
Contenuti sintetici
Le evoluzioni tecnologiche degli ultimi venti anni, da una parte hanno fornito al medico una serie di strumenti diagnostici non invasivi che gli consentono di ottenere importanti e, fino a poco tempo fa, non immaginabili informazioni circa la struttura e la funzione del cuore. D’altra parte anche la metodica invasiva ha subito un importante sviluppo.
Contenuto di questo corso sarà l’approccio alle metodologie non invasive per lo studio della struttura e funzione del cuore: ultrasonografia ecocolor doppler cardiaca, tecniche di medicina nucleare (scintigrafia, PET e teniche ibride SPECT/CT e PET/CT) risonanza magnetica cardiaca e TC coronarica. Verrà inoltre mostrato uno studio angiografico invasivo specificandone le potenzialità diagnostico-terapeutiche. Le metodiche si intendono contestualizzate ai fini dello studio delle principali alterazioni cardiache e vascolari acute e croniche e con particolare riferimento alle indicazioni all'esame nell'ipertensione arteriosa, alla cardiopatia ischemica e allo scompenso cardiaco.
Saranno inoltre trattate alcune specificità di genere in merito all'imaging cardiovascolare. In particolare, analizzare e valutare le patologie con un approccio medico e scientifico in un’ottica di genere per migliorare non solo le conoscenze sui diversi aspetti alla base delle differenze ma anche l’adeguatezza dell’intervento sulla salute per stimolare una maggiore attenzione alla raccolta dei dati anamnestici, strumentali e laboratoristici in relazione al genere del paziente.

Le lezioni si terranno presso l'ospedale Niguarda in modo da permettere la visione e l'uso delle metodiche in modo interattivo.

GIORNO 1: studio della struttura del cuore mediante metodica ultrasonografica.

GIORNO 2: TC coronarica e risonanza magnetica.

GIORNO 3: tecniche di medicina nucleare (scintigrafia, PET e tecniche ibride SPECT/CT e PET/TC).

GIORNO 4: studio delle coronarie mediante esame angiografico e trattamento endovascolare di lesioni aterosclerotiche.
Prerequisiti
Aver frequentato le lezioni di cardiologia del terzo anno del corso PMC1.
Modalità didattica

GIORNO 1: 3 ore di didattica interattiva con spiegazione dell'ecocardiogramma durante la sua esecuzione da parte del tutor su personale dedicato.

GIORNO 2: 1 ore di didattica erogativa in presenza su coro-TC e risonanza magnetica ed 1 ora di didattica interattiva con discussione di immagini radiologiche.

GIORNO 3: 1 ore di didattica erogativa in presenza sulle tecniche di medicina nucleare ed 1 ora di didattica interattiva con discussione di immagini scintigrafiche.

GIORNO 4: 1 ora di didattica erogativa sulla coronarografia ed 1 ora di didattica interattiva con discussione di immagini coronarografiche.
Tutte le attività sono svolte in presenza.

Aims
Giving students the tools to learn theoretical and practical indications to invasive and non-invasive study of the heart with the most modern methods supported by the most recent guidelines.
Contents
Technological developments of the last twenty years have given on one hand new non-invasive diagnostic tools that allows to obtain important and, until recently, no imaginable information about the structure and function of te heart. On the other hand also the invasive methodology have known a rapid evolution.
Content of this course will be the approach to non-invasive methods for studying the structure and function of the heart: ultransound Echo Color Doppler, magnetic resonance, nuclear medicine technique (myocardial stress scintigraphy, and PET) coronary computer tomography. It will be shown also a cardiac angiography with treatment of coronary stenosis. The techniques are contextualized to the study of major cardiac acute and chronic diseases with particular interest to indications in hypertension, ischemic heart disease, and heart failure.
Furthermore, we will discuss gender issue regarding cardiovascular imaging. In particular, to analyze and evaluate with a medical and scientific approch and in a gender based view with the aim to improv not only the knowledge on aspect determining gener differences but also the appropriateness of medical intervention in order to give higher attention on anamnestic, instrumental and laboratory data in relatioship to patients gender.
Detailed program
The course will be done at the Niguarda Hospital in order to offer the possibility to interact with the methodology used.

DAY 1: study of the structure of the heart using ultrasound technique.

DAY 2: cardiac magnetic resonance and coronary CT.

DAY 3: nuclear medicine technique (myocardial scintigraphy, PET, SPECT/CT and PET/CT).

DAY 4: study of coronary angiography with possible means of endovascular treatment of coronary stenosis.
Prerequisites
To have already attend the cardiological lessons of the PMC1 course.
Teaching form

DAY 1: 3 hours od interactive teaching with explanation direclty during the echocardiography examination done by the tutor.

DAY 2: 1 hour of didattic teaching on cardiac magnetic resonance and coronary CT and 1 hour of interactive teaching with discussion of radiological images.

DAY 3: 1 hour of didattic teaching on nuclear medicine technique and 1 hour of interactive teaching with discussion of scintigraphyc images.

DAY 4: 1 hour of didattic teaching on coronary angiography and 1 hour of interactive teaching with discussion of coronarographic images.
All the activities are on-site.
