Obiettivi

• Conoscere le principali caratteristiche dell’invecchiamento e la gestione delle problematiche legate alla cronicità della persona anziana

• Contestualizzare il concetto di fragilità e disabilità nell’anziano e l’importanza della valutazione multidimensionale (VMD) attraverso la conoscenza delle principali scale di valutazione

• Identificare le principali manifestazioni, dati, segni e sintomi della persona fragile e con disabilità

• Identificare le principali diagnosi infermieristiche, gli outcome e gli interventi infermieristici all’interno del percorso assistenziale intraospedaliero e territoriale.

• Identificare le principali diagnosi infermieristiche, gli outcome e gli interventi alle persone con patologie dell'apparato locomotore.

Contenuti sintetici

Il corso si prefigge, di fornire allo studente le conoscenze e le modalità necessarie all’identificazione e alla risposta ai bisogni di assistenza infermieristica della persona anziana, utilizzando il processo di assistenza e gli strumenti della valutazione multidimensionale, contestualizzandoli alle situazioni assistenziali in ambito geriatrico, ortopedico e riabilitativo, in ogni setting assistenziale.

Programma esteso

I contenuti delle lezioni si prefiggono di:
• Descrivere il quadro clinico delle principali patologie e sindromi geriatriche legate alla persona anziana che possono intervenire sull'organismo alterandone l'equilibrio
• Sapere riconoscere i meccanismi fisiopatologici che li determinano e indicarne l'approccio terapeutico/assistenziale
• Conoscere la manifestazione delle patologie trattate attraverso i dati, segni e sintomi in quanto riconoscibili come le principali variabili utili ad identificare le diagnosi infermieristiche
ARGOMENTI TRATTATI:
L’anziano fragile: inquadramento e riflessione attraverso i capisaldi della geriatria e accenni del Chronic Care Model. La valutazione multidimensionale dell’anziano fragile: metodi e strumenti.
Accenni di Self care nella persona anziana con patologia cronica.
Deperimento cognitivo, gestione delle diverse forme di demenza e del delirium nelle diverse manifestazioni.
Problematiche legate all'alterazione o rischio di alterazione dei processi legati all’alimentazione e idratazione, alla funzionalità dell’apparato urinario e intestinale, all'apparato locomotore nell’anziano.
Gestione delle problematiche ortogeriatriche
Gestione del dolore, dei rischi infettivi e delle patologie legate al cavo orale
Gestione del rischio di cadute, sindrome da immobilizzazione, e sarcopenia.
Educazione terapeutica e gestione del trattamento farmacologico

Definiti dal regolamento didattico.

Modalità didattica

Le lezioni sono svolte prevalentemente in modalità erogativa integrando alcune lezioni con esercitazioni individuali e/o di gruppo, video didattici, e contributi narrativi. Tutte le attività sono svolte in presenza.

INFERMIERISTICA DELLA DISABILITA’
Prova scritta Infermieristica: 14 test a risposta chiusa a scelta multipla di cui una sola è corretta e quattro domande aperte relative ad un breve caso clinico assistenziale inerente ad argomenti trattati in aula.
Per il superamento del modulo occorre rispondere in modo congruente ed esaustivo alle domande aperte valutate con punteggio da 0 a 4 per ogni singola domanda (cut-off 8 per superamento domande aperte) e 1 punto per ogni risposta corretta alle domande a risposta chiusa (cut-off 9): Superamento modulo di Infermieristica 17 punti totali. Non sono previste prove in itinere.

Aims

• Know the main characteristics of aging and the management of problems related to the chronicity of the elderly person

• Contextualize the concept of fragility and disability in the elderly and the importance of multidimensional assessment (VMD) through knowledge of the main assessment scales

• Identify the main manifestations, data, signs and symptoms of the fragile and disabled person

• Identify the main nursing diagnoses, outcomes and nursing interventions within the intrahospital and territorial care path.

• Identify the main nursing diagnoses, outcomes and interventions to people with pathologies of the locomotor system.

Contents

The course aims to provide the student with the necessary knowledge and modalities to identify and respond to the nursing needs of the elderly person, using the assistance process and tools of multidimensional assessment, contextualizing them to the care situations in the geriatric, orthopedic and rehabilitation, in every healthcare setting.

Detailed program

The contents of the lessons aim to:
• Describe the clinical picture of the main pathologies and geriatric syndromes related to the elderly person that may affect the body by altering its balance
• Know how to recognize the physiopathological mechanisms that determine them and indicate the therapeutic/ care approach
• To know the manifestation of the pathologies treated through the data, signs and symptoms as recognizable as the main variables useful to identify nursing diagnoses
SUBJECTS DEALT WITH:
The elderly fragile: framing and reflection through the cornerstones of geriatrics and hints of the Chronic Care Model. The multidimensional assessment of the elderly fragile: methods and tools.
Self care hints in the elderly person with chronic pathology.
Cognitive decay, management of different forms of dementia and delirium in different manifestations.
Problems related to the alteration or risk of alteration of the processes related to nutrition and hydration, the functionality of the urinary and intestinal tract, the locomotor system in the elderly.
Management of orthogeriatric problems
Management of pain, infectious risks and oral diseases
Risk management of falls, immobilization syndrome, and sarcopenia.
Therapeutic education and management of drug treatment

The lessons are carried out mainly in a paying mode integrating some lessons with individual and/or group exercises, teaching videos, and narrative contributions. All activities are carried out in presence

Assessment method

NURSING OF THE DISABLED
Written Nursing Test: 14 multiple choice closed-ended tests, of which only one is correct and four open-ended questions related to a short clinical care case related to topics dealt with in the classroom.
To pass the module it is necessary to answer in a congruent and exhaustive way the open questions evaluated with score from 0 to 4 for each individual question (cut-off 8 for passing open questions) and 1 point for each correct answer to closed-ended questions (cut-off 9): Passing Nursing module 17 total points. There are no ongoing to starts.
