Obiettivi
Il corso fornirà le conoscenze di base dell'immunologia. L'obiettivo del corso è quello di fornire le conoscenze dei meccanismi strutturali e molecolari del sistema immunitario. Il sistema immunitario è un insieme integrato di molecole e cellule che lavorano in modo coordinato per mantenere l'omeostasi del corpo e proteggerlo da agenti estranei, come i microrganismi ei loro prodotti. Il corso affronterà la dinamica e la complessità delle risposte immunitarie umorali e cellulo-mediate. Al termine del corso lo studente sarà in grado di descrivere: l'organizzazione della risposta immunitaria nei confronti dei diversi tipi di insulti e le peculiarità delle diverse componenti del sistema immunitario.

Programma esteso
Introduzione al sistema immunitario. Risposte naturali e adattative. Organizzazione anatomo-funzionale del sistema immunitario.
Immunità innata. Barriere anatomiche e difese chimiche iniziali. Il sistema del complemento e l'immunità innata. Pattern recognition da parte delle cellule del sistema immunitario innato. Riconoscimento delle strutture molecolari da parte delle cellule del sistema immunitario innato. Molecole effettrici solubili dell'immunità innata: il complemento, pentrassine, collectine e ficoline. Le risposte innate indotte contro l'infezione. Risposta infiammatoria: principali citochine proinfiammatorie, reclutamento leucocitario, ingestione e uccisione di microrganismi da parte dei fagociti, effetti sistemici e patologici dell'infiammazione. Risposta antivirale. Meccanismi di regolazione della risposta immunitaria innata.
Riconoscimento dell'antigene: Riconoscimento dell'antigene da parte dei recettori delle cellule B e delle cellule T. La struttura di una tipica molecola anticorpale. L'interazione della molecola dell'anticorpo con l'antigene specifico. Riconoscimento dell'antigene da parte delle cellule T.
Generazione di recettori per l'antigene dei linfociti.
Riarrangiamento genico delle immunoglobuline primarie. Riarrangiamento del gene del recettore delle cellule T. Variazione strutturale nelle regioni costanti dell'immunoglobulina. Evoluzione della risposta immunitaria adattativa.
Presentazione dell'antigene ai linfociti T. La generazione di ligandi del recettore delle cellule T α:β. Il complesso maggiore di istocompatibilità e la sua funzione.
Sviluppo e sopravvivenza dei linfociti. Sviluppo dei linfociti B. Sviluppo dei linfociti T. Selezione positiva e negativa delle cellule T.
Risposte immunitarie umorali. Attivazione delle cellule B da parte dell'antigene e delle cellule T helper. Le distribuzioni e le funzioni delle classi di immunoglobuline. La distruzione di agenti patogeni rivestiti di anticorpi tramite recettori Fc.
Immunità T cell-mediata. Siti per l'inizio delle risposte immunitarie adattative. Innesco di cellule T naive da parte di cellule dendritiche attivate da agenti patogeni. Proprietà generali delle cellule T effettrici e delle loro citochine. Citotossicità mediata da cellule T.

Le conoscenze acquisite saranno oggetto di prove in itinere composte da 36 quiz a risposta multipla e 1 domanda aperta. Verrà valutato il livello di conoscenza generale degli argomenti trattati durante le lezioni. Il superamento delle prove avverrà secondo le modalità descritte nel Syllabus Generale di "Basic Pathology".

Aims
The course will give the basic knowledge of immunology. The aim of the course is to give the knowledge of the structural and molecular mechanisms of the immune system. The immune system is an integrated set of molecules and cells that work in a coordinated way to maintain the body's homeostasis and protect it from foreign agents, such as microorganisms and their products. The course will address the dynamics and complexity of humoral and cell-mediated immune responses. At the end of the course the student will be able to describe: the organization of the immune response towards the different types of insults and the peculiarities of the different components of the immune system.

Detailed program
Introduction to the immune system. Natural and adaptive responses. Anatomical-Functional Organization of the Immune System.
Innate immunity. Anatomic barriers and initial chemical defenses. The complement system and innate immunity. Pattern recognition by cells of the innate immune system. Recognition of molecular structures by the cells of the innate immune system. Soluble effector molecules of innate immunity: the complement, Pentrassins, collectins and ficolins. The innate responses induced against infection. Inflammatory response: main proinflammatory cytokines, leukocyte recruitment, ingestion and killing of microorganisms by phagocytes, systemic and pathological effects of inflammation. Antiviral response. Mechanisms of regulation of the innate immune response.
Antigen Recognition. Antigen Recognition by B-cell and T-cell Receptors. The structure of a typical antibody molecule. The interaction of the antibody molecule with specific antigen. Antigen recognition by T cells.
Generation of Lymphocyte Antigen Receptors. Primary immunoglobulin gene rearrangement. T-cell receptor gene rearrangement. Structural variation in immunoglobulin constant regions. Evolution of the adaptive immune response.
Antigen Presentation to T lymphocytes. The generation of α:β T-cell receptor ligands. The major histocompatibility complex and its function.
Development and Survival of Lymphocytes. Development of B lymphocytes. Development of T lymphocytes. Positive and negative selection of T cells.
Humoral Immune Responses. B-cell activation by antigen and helper T cells. The distributions and functions of immunoglobulin classes. The destruction of antibody-coated pathogens via Fc receptors.
T cell-mediated immunity. Sites for the initiation of adaptive immune responses. Priming of naive T cells by pathogen-activated dendritic cells. General properties of effector T cells and their cytokines. T-cell-mediated cytotoxicity.

Assessment method
The knowledge acquired will be evaluated with an itinere test consisting of 36 multiple choice questions and 1 open question. The General knowledge of the topics covered during the lessons will be assessed. The tests will be considered passed according to the indications described in the General Syllabus of "Basic Pathology".
