Obiettivi

Il modulo costituisce l'ultimo elemento di continuità per quanto riguarda gli aspetti della formazione legati alle "medical humanities", di cui fanno parte il corso integrato "Humanities" del primo anno, il corso "Medicine and Society" del secondo anno, alcuni elettivi degli anni 3-6

Al termine del corso lo studente sarà in grado di conoscere e distinguere gli ambiti di applicazione e di efficacia dei differenti interventi psicologici (supporto psicologico, psicoeducazione, counselling e psicoterapia) ed effettuare appropriati invii specialistici; lo studente sarà altresì in grado di riconoscere l’eventuale appropriatezza dell’intervento psicologico nelle situazioni di patologia organica ed attivare le risorse psicologiche ospedaliere e territoriali adeguate; lo studente al termine delle esercitazioni pratiche acquisirà inoltre la capacità di autovalutazione della propria competenza professionale e dei cambiamenti psicologico attitudinali, in particolare per quanto riguarda l'acquisizione del ruolo professionale e la capacità di regolare adeguatamene il proprio comportamento e la relazione con i pazienti e con i colleghi.

Contenuti sintetici

La psicologia clinica nel sistema sanitario; gli interventi psicologico clinici Evidence Based; le differenze tra i vari tipi di intervento psicologico: supporto psicologico, psicoeducazione, counselling e psicoterapia individuale e i vari tipi di setting (individuale, di coppia di gruppo); i principali approcci psicoterapeutici (cognitivo, cognitivo-comportamentale, psicodinamico) trattamenti specifici per il trauma. Fattori psicologici di rischio e fattori protettivi nel modello bio-psico-sociale e nel trattamento. Verrà approfondita la relazione tra l'orientamento patient-centredness, la teoria dell'attaccamento e le organizzazioni di significato personale della teoria cognitivo-costruttivista.

I servizi ospedalieri di psicologia clinica, struttura, organizzazione e funzioni;

Gli interventi di psicologia clinica in ambito ospedaliero e nelle patologie psichiatriche;

Supporto psicologico: caratteristiche e ambiti di applicazione

Interventi psicoeducativi: caratteristiche e ambiti di applicazione

Psicoterapia: caratteristiche e ambiti di applicazione

Modelli di psicoterapia ed interventi Evidence Based

Trattamenti cognitivo-comportamentali,

Trattamenti cognitivisti dalla prima alla terza onda (cognitivismo classico, costruttivista, ACT, Compassion Therapy, Schema Therapy, Dialectic Behavior Therapy, Mindfullness Based Stress Reduction)

Il ruolo dell'attaccamento e della regolazione delle emozioni come fattore protettivo e/o di rischio psicopatologico.

La teoria cognitiva-costruttivista in relazione con la teoria dell'attaccamento e la patient-centredness.

Le organizzazioni di significato personale.

Esperienze di tirocinio medico in ambito ospedaliero

modulo Communication skills (corso Medicina e società, II anno) o equivalenti

Modalità didattica

Lezioni frontali, con utilizzo di metodologie attive e interattive (e.g. mentimeter, flipped classroom, buzz groups), discussione guidata in aula, con fruizione di video didattici e spezzoni di film.

Le esercitazioni pomeridiane a piccolo gruppo sono organizzate secondo le linee guida della Medical Education: attività riflessive su diversi tipi di contenuti per favorire l’acquisizione dell’atteggiamento professionale e una maggiore consapevolezza delle dinamiche individuali; discussione di casi clinici, simulazioni di interazioni cliniche .

Discussione degli argomenti trattati a lezione con domande aperte per valutare la capacità di individuare gli aspetti psicologici nelle situazioni di patologia organica o individuare le appropriate modalità di intervento psicologico-clinico nei casi di patologia organica o psichiatrica (in base alla gravità).

Aims

This module is the last step in the curriculum of the "medical humanities" pillar that include: first year "Humanities" integrated course, second year integrated course "Medicine and Society", some electives from years 3-6.

To know and distinguish the areas of application and effectiveness of the different psychological interventions (counseling/psychotherapy); to recognize the appropriateness of the psychological interventions in medical situations and be able to use the psychological services hospital and territorial; self-evaluation competence in the development of one's own professional competence and attitudinal psychological changes and patient-doctor relationship, teamwork, leadership and negotiation.

At the end of the course the student will be able to know and distinguish the areas of application and effectiveness of the different psychological interventions (psychological support, psychoeducation, counseling and psychotherapy) and and to give appropriate indication for specialist addressing; the student will also be able to recognize the possible appropriateness of the psychological intervention in situations of organic pathology and activate the appropriate hospital and territorial psychological resources; at the end of the practical small group activities, the student will also acquire the ability to self-evaluate his/her professional competence and psychological aptitude changes, in particular those regarding the acquisition of the professional role and the ability to adequately regulate his/her own behavior in the patient-doctor relationship, in teamwork activities, in the proper management of leadership, conflicts resolution and negotiation.

Contents

Clinical psychology in the health system; Evidence Based clinical psychology interventions; counseling and psychotherapy; different psychotherapeutic approaches; risk and protective factor in the bio-psycho-social approach to psychopathology and treatment.

Clinical psychology in the Italian health system; Evidence Based clinical psychological interventions; Different types of psychological intervention: psychological support, psychoeducation, counseling and psychotherapy and the various types of settings (individual, group couple); the main psychotherapeutic approaches (cognitive, cognitie-behavioral, psychodynamic) PTSD treatment. Psychological risk factors and protective factors in the bio-psycho-social model and treatment. The relationship between the patient-centredness disposition, the attachment theory and the personal organization meaning of the cognitive-constructivist theory will be explored.

Hospital services of clinical psychology: structure, organization and functions; clinical psychology interventions in hospital setting and in psychiatric disorders; psychological counselling interventions: characteristics and areas of application; psychotherapy models; Evidence Based interventions: cognitive behavioral treatments, DialecticBehavior Therapy, Mindfulness Based Stress Reduction, ACT. Attachment as a protective or risk factor in some of the most common clinical situations.

Definition and fields of application of clinical psychology

Hospital services of clinical psychology, structure, organization and functions;

Clinical psychology interventions in hospitals and psychiatric diseases;

Psychological counseling: features and areas of application;

Psychological support: characteristics and areas of application

Psychoeducational interventions: characteristics and areas of application

Psychotherapy: characteristics and areas of application

Psychotherapy: different approaches and Evidence Based interventions

Cognitive-behavioral treatments,

Cognitive treatments from the first to the third wave (classical cognitive therapy, constructivist cognitive therapy, ACT, Compassion Therapy, Schema Therapy, Dialectic Behavior Therapy, Mindfullness Based Stress Reduction)

The role of attachment and regulation of emotions as a protective and / or psychopathological risk factor.

Cognitive-constructivist theory in relation to attachment theory and patient-centredness.

Organizations of personal significance.

Communication skills module (Medicine and society course, 2nd year) or equivalent

Teaching form

Frontal lessons, with the use of active and interactive methodologies (e.g. mentimeter, flipped classroom, buzz groups), guided discussion in the classroom, with the use of educational videos and film clips.

Small group afternoon activities are organized according to the Medical Education guidelines: reflective activities to encourage the acquisition of professional attitude and a greater awareness of individual dynamics; discussion of clinical cases, simulations of clinical interactions.
