Obiettivi

Fornire le conoscenze di base relative alle più importanti modalità di imaging ed al loro utilizzo clinico nel contesto della radiologia gastroenterologica. Il ruolo della radiologia convenzionale e tomografica, incluse le tecniche ibride verrà illustrato. Il principale scopo del corso è documentare il potenziale delle diverse tecniche di imaging nella gestione delle patologie, in elezione e in urgenza. La trattazione seguerà uno schema strutturato per ambito anatomico.

  1. Conoscenza e capacità di comprensione
    Lo studente dovrà:
  • conoscere i principi fisici di base delle principali tecniche di imaging
  • comprendere le indicazioni cliniche delle metodiche radiologiche, nucleari e radioterapiche
  • acquisire conoscenze generali delle principali applicazioni cliniche

Contenuti sintetici

Valutare l’efficacia della radiologia convenzionale digitale, dell'ecografia e delle varie tecniche tomografiche digitali, quali TC e Risonanza magnetica nella determinazione diagnostica della patologia, con particolare riferimento all'appropriatezza.

Programma esteso

Ruolo della radiologia convenzionale nella patologia gastroenterica in elezione e in emergenza-urgenza. Valore di TC ed RM come stazioni diagnostiche fondamentali nella gestione della patologia infiammatoria, degenerativa e neoplastica nel distretto gastroenterico. Verranno date informazioni di base per quanto concerne l’interpretazione delle immagini, nei contesti clinici più comuni. Selezione della modalità di diagnostica per immagini in base al quesito clinico, secondo principi di appropriatezza diagnostica. Verranno introdotte le tecniche avanzate per la quantificazione della rigidità e della steatosi epatica. Verranno presentati i diversi mezzi di contrasto con le relative specificità.
Gli argomenti trattati includeranno l'approfondimento delle metodiche di imaging e l'interpretazione delle immagini per:

  • patologia benigna e maligna del tratto gastroenterico, del colon e del retto.
  • patologia benigna e maligna delle vie biliari e del pancreas
  • fegato: lesioni incidentali benigne, patologia cronica e lesioni maligne; protocolli di screening
  • cirrosi e ipertensione portale
  • trattamenti loco-regionali in radiologia interventistica

Prerequisiti

Conoscenze di base di anatomia umana, fisiologia e patologia generale.

Modalità didattica

5 lezioni frontali da 2 ore svolte in modalità erogativa in presenza.

Modalità di verifica del profitto e valutazione

Esame orale di circa 20-30 minuti. L’esame risulta favorevole se viene raggiunto un punteggio pari o superiore a 18/30. Discussione orale con approfondimento di vari argomenti. Le domande verranno concepite al fine di indurre lo studente a ragionare sul percorso diagnostico atto ad inquadrare la problematica clinica discussa.

Aims

Provide basic knowledge of the most important imaging modalities and their clinical use in the context of gastrointestinal radiology. The role of conventional and tomographic radiology, including hybrid techniques, will be illustrated. The main aim of the course is to document the potential of the different imaging techniques in the management of diseases, both elective and emergency. The discussion will follow a structured approach based on anatomical area.

  • understand the clinical indications of radiological, nuclear medicine, and radiotherapy methods
  • acquire general knowledge of the main clinical applications

  • identify the most appropriate imaging method
  • provide a general interpretation of imaging findings
  • understand the role of diagnostic imaging in clinical pathways

  • assess the benefits and risks of the different methods
  • consider aspects of appropriateness and radiation protection
  • develop a critical approach to requesting diagnostic examinations
  1. Communication skills
    The student will be able to:
  • use appropriate medical terminology in the field of radiology
  • correctly communicate basic indications and results
  • interact with diagnostic imaging specialists

  • develop the ability to update knowledge independently
  • consult and understand review articles
  • integrate theoretical and clinical knowledge

Contents

Assess the effectiveness of digital conventional radiology, ultrasound, and the various digital tomographic techniques, such as CT and magnetic resonance imaging, in the diagnostic assessment of disease, with particular reference to appropriateness.

Detailed program

Role of conventional radiology in gastrointestinal diseases in elective and emergency-urgent settings. Value of CT and MRI as fundamental diagnostic tools in the management of inflammatory, degenerative, and neoplastic diseases of the gastrointestinal tract. Basic information will be provided regarding image interpretation in the most common clinical contexts. Selection of the imaging modality based on the clinical question, according to principles of diagnostic appropriateness. Advanced techniques for the quantification of liver stiffness and hepatic steatosis will be introduced. The different contrast agents will be presented, together with their specific characteristics.
The topics covered will include an in-depth discussion of imaging methods and image interpretation for:

  • benign and malignant diseases of the gastrointestinal tract, colon, and rectum.
  • benign and malignant diseases of the biliary tract and pancreas
  • liver: benign incidental lesions, chronic disease, and malignant lesions; screening protocols
  • cirrhosis and portal hypertension
  • loco-regional treatments in interventional radiology

Prerequisites

Basic knowledge of human anatomy, phisiology, and general pathology.

Teaching form

5 two-hour lectures delivered in person in lecture-based format.

Assessment method

Oral exam conducted in approximately 20-30 minutes. The exam is positively evaluated with a score of 18/30 or higher. Oral discussion with possible deepening of one or more topics. The questions proposed will be constructed in such a way to induce the student to understand the clinical use and significance of any of the most relevant imaging modalities.
