Obiettivi

Il modulo mira a fornire agli studenti conoscenze teoriche e pratiche, nonché competenze applicabili riguardanti l’uso delle tecniche di medicina nucleare e radioterapia nelle neoplasie ematologiche e malattie correlate, integrando e approfondendo le basi cliniche acquisite nel corso di oncoematologia e nelle clerkship correlate.
Obiettivi Formativi
Conoscenze Teoriche:

  1. Comprendere i principi fondamentali della medicina nucleare diagnostica, inclusi i principali radiofarmaci impiegati nelle malattie ematologiche come linfomi, leucemie e mieloma multiplo.
  2. Conoscere le tecniche più comuni di imaging nucleare (es. PET/TC con FDG, scintigrafia ossea) e le loro applicazioni nella diagnosi, stadiazione, monitoraggio e valutazione della risposta nelle malattie oncoematologiche.
  3. Familiarizzare con le applicazioni terapeutiche della medicina nucleare, in particolare la radioimmunoterapia e altre terapie radionuclidiche specifiche per neoplasie ematologiche.
  4. Comprendere i principi della radioterapia ematologica, con particolare attenzione alla radioterapia a sito interessato, alla radioterapia totale corporea (TBI) e alle tecniche avanzate (IMRT, IGRT, protonterapia).
  5. Approfondire aspetti regolatori, di sicurezza e farmacologici relativi all’impiego di radiofarmaci e tecnologie di radioterapia specifici per l’oncoematologia.
    Competenze Pratiche e Interpretative:
  6. Saper interpretare correttamente i referti di medicina nucleare relativi a neoplasie ematologiche (coinvolgimento linfonodale, malattia midollare, lesioni scheletriche).
  7. Conoscere indicazioni cliniche, protocolli e gestione del paziente sottoposto a terapie radionuclidiche e radioterapia nel percorso terapeutico oncoematologico.
  8. Applicare criteri appropriati per la prescrizione e interpretazione di indagini diagnostiche di medicina nucleare e radioterapia secondo linee guida cliniche.
  9. Collaborare efficacemente con ematologi, radiologi, medici nucleari e radioterapisti all'interno di un team multidisciplinare.
    Competenze Trasversali:
  10. Integrare dati clinici, biologici e di imaging per una valutazione completa del paziente con malattie ematologiche.
  11. Comunicare in modo chiaro con i pazienti riguardo le procedure diagnostiche e terapeutiche, inclusi rischi e benefici.
  12. Mantenere comportamenti professionali e collaborativi nei team multidisciplinari, assicurando il rispetto delle normative in materia di sicurezza.
  13. Sviluppare autonomia nell’esecuzione e supporto delle procedure diagnostiche e terapeutiche di medicina nucleare sotto supervisione.

Contenuti sintetici

Questo modulo ha l’obiettivo di esporre gli studenti ad una importante fase del patients’ journey del paziente oncologico, quando cioè viene sottoposto a tecniche di imaging -a scopo di screening, diagnosi, stadiazione, ristadiazione e monitoraggio della risposta al trattamento – e dei trattamenti avanzati di radiodiagnostica interventista, terapia radiometabolica e con radioligandi e radioterapia a fasci esterni.
E’, infatti, nostra convinzione che comprendere come si svolge questa esperienza sia fondamentale per tutti i medici che saranno chiamati a rispondere a quesiti specifici. Possedere il corretto background di informazioni è fondamentale per non creare false paure o false aspettative ed aumentare la compliance del paziente a questa fase del suo percorso diagnostico-terapeutico.
In particolare riteniamo che l’evoluzione della moderna medicina necessiti della comprensione di tutte le procedure, anche quelle molto specialistiche e ad alta complessità che fanno parte del trattamento integrato delle neoplasie. Non è infrequente, infatti, che i pazienti ricevano come parte della strategia terapeutica della loro neoplasia a diversi trattamenti integrati in una ottica sequenziale. Pertanto, conoscerne il rapporto rischio-beneficio aiuta a costruire questa strategia evitando di sprecare opportunità e ottimizzando le risorse disponibili.
Obiettivi Pratici al Termine della Clerkship

  1. Interpretare criticamente i referti diagnostici di medicina nucleare e radioterapia nell’ambito dell’oncoematologia.
  2. Comunicare efficacemente con pazienti e team multidisciplinari sulle terapie nucleari e radioterapiche.
  3. Applicare in sicurezza protocolli operativi e buone pratiche nella gestione dei trattamenti radionuclidici.

Programma esteso

Durante questo modulo gi studenti saranno esposti alle attività cliniche quotidiane di erogazione di esami diagnostici a scopo di screening, diagnosi, stadiazione, ristadiazione e monitoraggio della risposta al trattamento ed a sessioni di trattamenti avanzati di radiodiagnostica interventista, terapia radiometabolica e con radioligandi e radioterapia a fasci esterni.
In questo modo potranno avere le conoscenze di base per poter rispondere alle principali domande a cui tutti i medici sono chiamati a rispondere quando propongono un test di immagine ad un paziente.

Introduzione Pratica alla Medicina Nucleare Diagnostica in Oncoematologia
Osservazione e partecipazione attiva nella somministrazione di radiofarmaci diagnostici (es. ^18F-FDG per linfomi) e nell’esecuzione di esami di medicina nucleare (PET/TC, scintigrafia ossea).
Affiancamento ai medici nucleari durante la preparazione del paziente e l’esecuzione dell’esame, con attenzione a protocolli, sicurezza e comunicazione con il paziente.
Sessioni guidate di interpretazione delle immagini, correlando la storia clinica con i dati radiologici.
Discussioni di casi clinici reali integrate con contributi multiprofessionali.
**Apprendimento Pratico in Terapie con Radionuclidi

Partecipazione diretta alle terapie radionuclidiche specifiche per neoplasie ematologiche, come la radioimmunoterapia (es. anti-CD20).
Supporto nel monitoraggio clinico dei pazienti trattati e gestione degli effetti collaterali, sotto supervisione.
Partecipazione a riunioni multidisciplinari per la pianificazione e valutazione delle terapie.
Introduzione alle Tecniche di Radioterapia in Oncoematologia**
Affiancamento in reparto radioterapico durante simulazione, pianificazione e somministrazione dei trattamenti (inclusa TBI).
Comprensione del ruolo dell’imaging integrato per la pianificazione e il monitoraggio della radioterapia.
Interazione con radioterapisti per valutare indicazioni cliniche e profili rischio-beneficio.
Partecipazione alla gestione quotidiana del paziente in trattamento radioterapico.
Aspetti Pratici di Sicurezza e Normativa
Coinvolgimento diretto nelle procedure di sicurezza per la gestione di radiofarmaci e rifiuti radioattivi.
Applicazione pratica dei protocolli di sicurezza durante tutte le procedure nucleari e radioterapiche.
Briefing su aspetti normativi, documentazione e responsabilità professionali, arricchiti da esempi clinici reali.
Comunicazione e Interazione Professionale
Partecipazione attiva nelle interazioni medico-paziente durante le procedure di medicina nucleare e radioterapia, con attenzione alla comunicazione chiara ed empatica.
Osservazione e partecipazione a incontri multidisciplinari, con opportunità di discutere e riflettere sulle esperienze cliniche.
Attività formative sulla gestione efficace delle informazioni e comunicazioni con colleghi e operatori sanitari.

Prerequisiti

Questa clearkship necessita delle conoscenze di base di IMAGE DIAGNOSTICS e ONCO-HEMATOLOGICAL DISEASES.

Aims

This module aims to provide students with theoretical and practical knowledge and applicable skills concerning the use of nuclear medicine and radiotherapy techniques in hematological malignancies and related diseases, integrating and deepening the clinical foundations acquired during the oncohematology course and associated clerkships.
Educational Objectives
Theoretical Knowledge:

  1. Understand the fundamental principles of diagnostic nuclear medicine, including the main radiopharmaceuticals used for hematological diseases such as lymphomas, leukemias, and multiple myeloma.
  2. Learn about the most common nuclear imaging techniques (e.g., PET/CT with FDG, bone scintigraphy) and their applications in the diagnosis, staging, monitoring, and response assessment of oncohematologic diseases.
  3. Become familiar with therapeutic applications of nuclear medicine, particularly radioimmunotherapy and radionuclide therapies specific for hematological malignancies.
  4. Understand the principles of radiotherapy in hematology, focusing on involved site radiotherapy, total body irradiation (TBI) used in conditioning regimens, and advanced treatment techniques (IMRT, IGRT, proton therapy).
  5. Deepen knowledge of regulatory, safety, and pharmacological aspects related to the use of radiopharmaceuticals and radiotherapy technologies specific to oncohematology.
    Practical and Interpretative Skills:
  6. Interpret nuclear medicine imaging reports related to hematological malignancies (lymph node involvement, bone marrow disease, skeletal lesions).
  7. Understand clinical indications, protocols, and management of patients undergoing radionuclide therapies and radiotherapy as part of hematologic cancer treatment plans.
  8. Apply appropriate criteria for ordering and interpreting diagnostic nuclear medicine and radiotherapy procedures according to clinical guidelines for hematological diseases.
  9. Collaborate effectively with hematologists, nuclear medicine specialists, radiologists, and radiotherapists within a multidisciplinary oncohematology care team.
    Transversal Competences:
  10. Integrate clinical, biological, and imaging data for comprehensive evaluation of patients with hematological malignancies.
  11. Communicate clearly with patients regarding diagnostic and therapeutic procedures, including potential risks and benefits.
  12. Maintain professionalism and collaboration skills within multidisciplinary teams, ensuring compliance with safety and regulatory standards.
  13. Develop autonomous competency in assisting, performing, and documenting nuclear medicine diagnostic and therapeutic procedures under supervision.

Contents

This module aims to expose students to an important phase of the Patients' Journey of the cancer patient, when they are subjected to imaging techniques - for the purposes of screening, diagnosis, staging, restaging and monitoring the response to treatment - and advanced treatments of interventional radiodiagnostics, metabolic radiotherapy and radioligand therapy and external beam radiotherapy.
It is, in fact, our belief that understanding how this experience takes place is fundamental for all doctors who will be called upon to answer specific questions. Having the correct background information is essential in order not to create false fears or false expectations and increase the patient's compliance with this phase of his diagnostic-therapeutic path.
In particular, we believe that the evolution of modern medicine requires the understanding of all procedures, even those that are very specialized and highly complex, which are part of the integrated treatment of tumors. It is not uncommon, in fact, for patients to receive, as part of the therapeutic strategy of their neoplasm, several treatments integrated in a sequential perspective. Therefore, knowing the risk-benefit ratio helps to build this strategy, avoiding wasting opportunities and optimizing available resources
Practical Learning Objectives for Students at Clerkship Completion

  1. Demonstrate critical interpretation skills of diagnostic nuclear medicine and radiotherapy reports within hematologic oncology context.
  2. Exhibit effective communication with patients and multidisciplinary teams about nuclear medicine and radiation therapy treatments.
  3. Apply operational protocols and best practices for safe management of radionuclide treatments and radiotherapy in hematological diseases.

Detailed program

During this module, students will be exposed to the daily clinical activities of providing diagnostic tests for the purposes of screening, diagnosis, staging, restaging and monitoring of response to treatment and to advanced treatment sessions of interventional radiodiagnostics, metabolic radiotherapy and radioligand therapy and external beam radiotherapy.
In this way they will be able to have the basic knowledge to be able to answer the main questions that all doctors are asked to answer when they propose an image test to a patient.

Practical Introduction to Diagnostic Nuclear Medicine in Oncohematology
Observation and active participation in administering diagnostic radiopharmaceuticals (e.g., ^18F-FDG for lymphoma) and acquisition of nuclear imaging studies (PET/CT, bone scintigraphy).
Shadowing nuclear medicine physicians during patient prep and study execution with emphasis on protocols, patient safety, and communication.
Guided image interpretation sessions correlating clinical history with imaging findings.
Case discussions based on real patient data integrated with multidisciplinary clinical input.
Hands-On Learning in Radionuclide Therapy
Direct participation in radionuclide therapies specific to hematologic malignancies, such as radioimmunotherapy (e.g., anti-CD20 agents).
Clinical monitoring of treated patients under supervision including management of side effects.
Attendance in multidisciplinary meetings planning radio-therapeutic approaches and reviewing outcomes.
Introduction to Radiotherapy Techniques in Oncohematology
Observation in radiotherapy departments during simulation, treatment planning, and delivery (including TBI).
Understanding the role of integrated imaging for radiation planning and monitoring.

Prerequisites

This clearkship required the basic knowledge of IMAGE DIAGNOSTICS and ONCO-HEMATOLOGICAL DISEASES.
