Obiettivi

Il modulo di Fisiologia gastro-intestinale si propone di fornire allo studente le conoscenze fondamentali sui meccanismi funzionali che regolano l’attività dell’apparato digerente. Al termine del modulo, lo studente dovrà conoscere l’organizzazione funzionale del tratto gastro-intestinale, i meccanismi di secrezione, motilità, digestione e assorbimento, nonché il controllo nervoso e ormonale delle principali funzioni digestive. Particolare attenzione sarà dedicata al ruolo del sistema nervoso enterico, all’elettrofisiologia della muscolatura liscia gastro-intestinale, alla regolazione delle secrezioni salivare, gastrica, pancreatica, intestinale ed epatica, e ai processi fisiologici alla base dei sensi chimici del gusto e dell’olfatto.

Contenuti sintetici

Il modulo affronta le funzioni generali dell’apparato digerente, la struttura funzionale del tratto gastro-intestinale, i meccanismi di secrezione e motilità, il controllo nervoso e ormonale della funzione digestiva, con particolare riferimento al sistema nervoso enterico e ai principali ormoni gastro-intestinali. Verranno inoltre trattate la fisiologia della cavità orale, la secrezione salivare, la fisiologia del gusto e dell’olfatto, la secrezione gastrica, intestinale, pancreatica ed epatica, e i processi di digestione e assorbimento dei nutrienti.

Programma esteso

Il modulo introduce le funzioni e le caratteristiche generali dell’apparato digerente, con riferimento all’organizzazione funzionale del tratto gastro-intestinale e al ruolo coordinato di motilità, secrezione, digestione e assorbimento. Saranno descritte le principali caratteristiche strutturali e funzionali della parete gastro-intestinale e il loro significato fisiologico nei diversi segmenti del tubo digerente.

Una parte del modulo sarà dedicata ai meccanismi di controllo della funzione gastro-intestinale, con particolare attenzione al sistema nervoso enterico, al controllo nervoso autonomo e alla regolazione ormonale. Verranno analizzati i principali meccanismi che regolano la motilità gastro-intestinale, l’elettrofisiologia delle cellule muscolari lisce del tratto gastro-intestinale e il ruolo degli ormoni gastro-intestinali, tra cui gastrina, colecistochinina e secretina.

Saranno quindi trattate le funzioni della cavità orale, la secrezione salivare e la fisiologia dei sensi chimici, con riferimento ai meccanismi alla base del gusto e dell’olfatto. Verranno descritti la composizione della saliva, la regolazione della secrezione salivare e il ruolo della cavità orale nelle prime fasi della digestione.

Il modulo proseguirà con lo studio dello stomaco e della secrezione gastrica, includendo composizione, meccanismi di secrezione e regolazione della secrezione acida ed enzimatica. Saranno inoltre affrontate le secrezioni intestinali, la secrezione pancreatica e la secrezione epatica, con riferimento alla loro composizione, regolazione e ruolo nei processi digestivi.

Infine, verranno analizzati i meccanismi di digestione e assorbimento dei principali nutrienti, considerando l’integrazione funzionale tra secrezioni digestive, motilità intestinale e trasporto attraverso l’epitelio intestinale.

Prerequisiti

Le propedeuticità da rispettare per sostenere l’esame di Fisiologia umana sono quelle indicate dal Regolamento Didattico di medicina e Chirurgia A.A. 2026/2027

Modalità didattica

Le lezioni da 2 ore ciascuna si svolgeranno in presenza, a meno di emergenze. Inizieranno con una prima parte in cui verranno esposti i concetti (modalità erogativa) per poi aprirsi ad un'interazione con gli studenti che definisce la parte successiva della lezione in modalità interattiva.
I metodi di insegnamento includeranno lezioni frontali, video e discussioni in classe.
Le attività didattiche potranno prevedere, fino a un massimo del 30% delle ore di lezione, l’utilizzo di modalità didattiche innovative, inclusa l’erogazione sotto la supervisione del docente, la copresenza di docenti e/o la didattica da remoto, secondo quanto previsto dal Decreto Ministeriale n. 1835 del 6 dicembre 2024.

Aims

The Gastrointestinal Physiology module aims to provide students with fundamental knowledge of the functional mechanisms regulating the activity of the digestive system. By the end of the module, students are expected to understand the functional organization of the gastrointestinal tract, the mechanisms of secretion, motility, digestion and absorption, as well as the neural and hormonal control of the main digestive functions. Particular attention will be devoted to the role of the enteric nervous system, the electrophysiology of gastrointestinal smooth muscle cells, the regulation of salivary, gastric, pancreatic, intestinal and hepatic secretions, and the physiological mechanisms underlying the chemical senses of taste and smell.

Contents

The module covers the general functions of the digestive system, the functional structure of the gastrointestinal tract, and the mechanisms of secretion and motility. It also addresses the neural and hormonal control of digestive function, with particular reference to the enteric nervous system and the main gastrointestinal hormones. The physiology of the oral cavity, salivary secretion, taste and smell, gastric, intestinal, pancreatic and hepatic secretions, and the processes of digestion and nutrient absorption will also be discussed.

Detailed program

The module introduces the general functions and main characteristics of the digestive system, with reference to the functional organization of the gastrointestinal tract and the coordinated role of motility, secretion, digestion and absorption. The main structural and functional features of the gastrointestinal wall and their physiological significance in the different segments of the digestive tract will be described.

Part of the module will focus on the mechanisms controlling gastrointestinal function, with particular attention to the enteric nervous system, autonomic neural control and hormonal regulation. The main mechanisms regulating gastrointestinal motility will be analysed, together with the electrophysiology of gastrointestinal smooth muscle cells and the role of gastrointestinal hormones, including gastrin, cholecystokinin and secretin.

The functions of the oral cavity, salivary secretion and the physiology of the chemical senses will then be addressed, with reference to the mechanisms underlying taste and smell. The composition of saliva, the regulation of salivary secretion and the role of the oral cavity in the early stages of digestion will be described.

The module will then cover the stomach and gastric secretion, including the composition, mechanisms and regulation of acid and enzymatic secretion. Intestinal secretions, pancreatic secretion and hepatic secretion will also be discussed, with reference to their composition, regulation and role in digestive processes.

Finally, the mechanisms of digestion and absorption of the main nutrients will be analysed, considering the functional integration among digestive secretions, intestinal motility and transport across the intestinal epithelium.

Prerequisites

The prerequisite requirements to be fulfilled in order to sit the Human Physiology exam are those indicated in the Teaching Regulations of the Medicine and Surgery degree programme for the 2026/2027 academic year.

Teaching form

All lessons of 2 hours each will be in person, except in emergencies. They will begin with an initial part where concepts will be presented (lecture-based mode), followed by interaction with students that defines the subsequent part of the lesson in interactive mode.
Teaching methods will include lectures, videos, and class discussions.
Teaching activities may include, up to a maximum of 30% of the total lecture hours, the use of innovative teaching methods, including delivery under the supervision of the lecturer, co-teaching arrangements and/or remote teaching, in accordance with Ministerial Decree No. 1835 of 6 December 2024.
