Obiettivi
il Corso BCS ha l'obiettivo di formare gli studenti sulla semeiologia di base in medicina interna, chirurgia ed emergenza.
i singoli moduli saranno dettagliati separatamente:
Medicina interna
Al termine del programma di Clerkship, lo studente deve essere in grado di:
- Approcciare adeguatamente il paziente (saper assumere atteggiamento e linguaggio in relazione alle condizioni di paziente e ambiente (ambulatorio, degenza, emergenza)
- Raccogliere correttamente un anamnesi
- Estrarre ed imputare dati clinici da data base elettronico, adottando una corretta terminologia semeiologica;
- Identificare sintomi e segni di anormalità / alterazione (interpretare i più comuni esami di laboratorio)
- Praticare correttamente le manovre semeiologiche generali sul paziente (effettuare l'esame obiettivo generale, addominale, cardiovascolare, toraco-polmonare
- Misurare la pressione arteriosa, rilevare la frequenza cardiaca periferica e centrale, effettuare saturimetria, eseguire un ECG;
- Iniziare ad applicare gli strumenti logici del ragionamento clinico nella valutazione critica di sintomi, segni clinici, e dati laboratoristici e strumentali;
Surgery
Al termine del corso/blocco di attività, lo studente avrà compreso la storia generale della chirurgia attraverso i
secoli e il ruolo attuale della chirurgia nella gestione generale delle patologie gastrointestinali.
Saranno inoltre in grado di:
· Interagire con il paziente
· Raccogliere l'anamnesi medica e chirurgica
· Adottare la terminologia adeguata per comunicare con i pazienti e con il personale sanitario
· Utilizzare un database elettronico per i dati clinici
· Individuare, esaminare e descrivere i principali sintomi e segni patologici in relazione agli aspetti chirurgici delle
patologie gastrointestinali. Evidenziare un approccio multidisciplinare, sfruttando il Metodo PBL.
Adottare le competenze teorico-pratiche adeguate per eseguire le prove di esame clinico di base in chirurgia
generale e nelle specialità chirurgiche.
Eseguire manovre chirurgiche di base come esame pelvico, cateterizzazione urinaria, suture cutanee

Descrivere i fondamenti, i meccanismi fisiopatologici, i quadri clinici, il monitoraggio e il trattamento delle principali
patologie urgenti mediche e chirurgiche.
Applicare i protocolli di rianimazione cardio-polmonare BLSD e ACLS
Contenuti sintetici
Il programma di Clerkship è disegnato per permettere agli studenti di sviluppare un approccio integrato al rapporto paziente-medico. Il focus sarà dedicato all’apprendimento delle tecniche semeiologiche di base, al fine di preparare lo studente al programma clinico.
Medicina interna
Il programma si fonda su 3 componenti maggiori:
1. Capacità verbali e relazionali (incluse abilità di comunicazione, Principi di raccolta di un anamnesi mirata: metodologia di approccio al paziente anziano, adulto e pediatrico e valutazione di genere. raccolta di anamnesi approccio età- e genere correlato). Rilievi anamnestici particolari (alterazioni della coscienza, dei sensi, segni di anormalità).
2. Conoscenze di base dell’esame obiettivo. Conoscenza dei principi di base dell’esame obiettivo generale e mirato per organi, con rilievi attesi ed inattesi (le quattro fasi: Ispezione, Palpazione, Percussione, Auscultazione). Identificazione dei quadri identificativi per l’identificazione di segni e sintomi caratteristici per la diagnosi dei problemi. Semeiotica generale del dolore. Alterazioni della temperatura. Alterazioni del sensorio.
3. Interpretazione dei Dati. (relazione patiente-medico, Segni e sintomi, valutazione dei risultati di test diagnostici. Capacità di estrarre ed imputare dati in cartella clinica elettronica e/o cartacea con terminologia medica appropriata. Preparazione di una relazione clinica (lettera dimissioni dopo visita ambulatoriale).

Il programma di Clerkship clinica prevede la frequenza a rotazione in differenti ambiti specialistici della medicina interna, adulto e pediatrico. Le attività definite saranno svolte in sessioni d’insegnamento, in sessioni pratiche, in autoapprendimento e su pazienti ambulatoriali e in degenza:
- Problem-based learning
- Case-based learning
- Sessioni tutoriali di esercitazioni pratiche tra studenti, ed infine su pazienti (degenza ed ambulatoriali)
- Sessioni di autoapprendimento
- Lezioni frontali
- Frequenza presso le strutture di degenza ed ambulatoriale
Gli studenti riceveranno feedback regolari durante tutto il tirocinio in base alle loro capacità di valutazione critica e di risoluzione dei problemi medici, nonché alla loro comprensione delle conseguenze terapeutiche e di eventuali ulteriori indagini. Durante la normale cura del paziente, agli studenti sarà richiesto di dimostrare queste abilità e incorporare questa conoscenza nelle loro decisioni

Prove “in itinere” dopo sessioni di case- e problem-based learning:
· Preparazione di saggio breve su tematiche svolte (anche lavoro di gruppo)
· L’apprendimento di competenze cliniche è verificato tramite test teorico/pratici chiamati OSCE (“esame clinico obiettivo strutturato”). In ciascun OSCE lo studente viene confrontato con una situazione clinica nella quale si richiede di eseguire una o più attività cliniche (p.e. raccolta anamnesi, esame obiettivo, test semeiologici, capacità di comunicazione, interpretazione di dati strumentali, decisioni cliniche). Ogni studente riceve di volta in volta un riscontro del risultato ottenuto e del punteggio totale dei vari OSCE sostenuti. Esercizi pratici su manovre semeiologiche (tra studenti e su paziente)
· Produzione di almeno un quiz a risposta chiusa (scelta multipla) mirato per ciascuna sessione di case- e problem-based learning
Prova finale:
· Test a risposte chiuse (30 domande a scelta multipla o dicotomica – 1 punto a domanda – livello minimo per il superamento 18/30): propedeutico all'esame pratico: anamnesi ed esame obiettivo del paziente
· Raccolta e valutazione globale degli elaborati prodotti in corso delle sessioni PBL/CBL
Il giudizio finale sarà: Idoneo /non Idoneo

The BCS course should provide adequate formation on the basic semeiology of internale medicine, Surgery and Emergency
each module shall be detailed underneath:
Internal Medicine
At the end of the clerkship program the student should be able to:
- adequately approach patient encounter (adapt attitude and language to both patient status and environment (out-patient, in-patient, emergency)
- Properly collect a medical history
- Imput and extract data from an electronic medical data base (proper use of medical language)
- Identify signs and symptoms of abnormality /alteration (interpretation of the most common biochemical tests)
- Practice the proper physical examination/maneuvers on patients (general examination, abdomen, cardiovascular and thorax-pulmonary)
- Blood pressure, central and peripheral heart rate measurement, Oximetry, ECG
- Apply the clinical reasoning based on signs, symptoms and laboratory findings

- Learning the basics and mechanisms of physiopathology, diagnosis, monitoring and treatment of clinical emergencies, medical and surgical.

At the end of the course/activity block, the student will have understood the general history of surgery through the
centuries and the current role of surgery in the general management of the GI diseases.
They will also be able to adapt to the surgical patient the following items:
· Approach the patient
· Harvest a medical and surgical history
· adopt the appropriate terminology to communicate with patients and surgical staff
· Use an electronic database for clinical data
· Identify, examine and describe the main pathological symptoms and signs in relation to the surgical aspects of GI
disorders. Highlight on a multidisciplinary approach, exploiting the PBL Method.
Adopt the appropriate theorical and practical skills to perform the basic clinical examination tests in general surgery
and surgical specialties.
Perform basic surgica maneuvers such as pelvic examination, urinary catheterisation, skin sutures
Contents
The medical Clerkship is designed to allow students to develop an integrated approach to the doctor-patient relationship.
In the Medical Clerkship, the focus is on learning core medical concepts and basic professional skills to prepare students for the Clinical program and beyond.
Internal Medicine
The clerkship program is based on 3 major components:
1. Verbal data-gathering (including communication skills, medical history-taking; Age-specific approach for infants, children, adolescents, and older adults, and the healthy female evaluation
2. Basic physical examination skills. Key physical exam steps (Inspection, Palpation, Percussion, Auscultation), along with expected and unexpected findings. clues for identifying characteristic symptoms and diagnosing patient problems
3. Data interpretation (patient-physician relationship, signs & symptoms, diagnostic tests findings evaluation). Reporting and documenting findings for electronic charting

The clinical clerkship program provides for rotational attendance in different specialist fields of internal medicine, both adult and pediatric. The defined activities will be carried out in teaching sessions, in practical sessions, in self-learning and on outpatient and inpatients:
- Problem-based learning (PBL)
- Case Based Learning (CBL)
- Practice sessions with puppets or among students/teachers
- Attending Out- and Inpatients hospital wards
- Frontal lectures
- Attendance of in patients and outpatients units
Students shall receive regular feedbacks throughout the clerkship according to their critical appraisal and medical problem-solving skills, as well as their understanding of therapeutic consequences and eventual further investigations. During normal patient care, students will be required to demonstrate these skills and incorporate this knowledge into their care decisions

Ongoing testing after each PBL/PCL:
· Short essay (including group-work) related to each PBL/PCL session
· The development of clinical skills is assessed by OSCE (Objective structured clinical examination). Each OSCE faces the student with a unique clinical case which will test particular skills such as history-taking, physical examination, practical tests/maneuvers, communication skills, test/data interpretation, medical decision-making. Each student receives feedback from the assessor as well as overall scores for each OSCE.
· The preparation , by the student, of a multiple choice quiz for each PBL/CBL session
Final test:
· Multiple choice quiz (30 questions – 1 point for question – minimal level for idoneity: 18/30, propedeutic to practical examination: formal clinical evaluation of a patient (history, physical examination)
· Collection of the single short papers/compositions
The final evaluation shall be: Pass /not Passed
